How to fill-up for freshers


Please read instruction before fill up the form
As Student
  •  Students can apply for New card/Card Re-Print.
  •  All fields are mendatory.
  •  Fill all fields correctly.
  •  Upload image(maxminum of 100kb) and admission reciept/renewal admission reciept(maximum 100kb).
  •  A confirmation email will be sent to your provided email along with acknowledgement number which is important for further process.
  •  Visit library for payment of 100.00 /-(applicable for card re-print) along with valid ATM card and acknowledgement number.
  • Documents :

  •  For new membership apply any admission reciept need to be upload in pdf/jpg format(maximum size 200kb) with visible clearity.
  •  For new card produce admission reciept (with acknowledgement no.) and card renewal only acknowledgement no. is necessary.
As CITK Staff
  •  Staff can apply for New card/Card Re-Print.
  •  All fields are mandatory.
  •  Upload image in jpg only(maximum of 100kb) and appointment letter in jpg/pdf format(maximum 200kb).
  •  A confirmation email will be sent to your provided email along with acknowledgement number which is important for further process.
  • Documents :

  •  For new card an applicant must be submit a copy of appointment letter (with acknowledgement no.) and card renewal only acknowledgement no. is necessary.
As Guest
  •  Central Library also provides guest membership.
  •  It is necessary to read the rules and regulations for guest membership before filing up online application.
  •  Guest member can apply for New card/Card Re-Print.
  •  All fields are mandatory.
  •  For guest membership, person should have a valid CITK family member as referee.
  •  Upload imagein jpg only(maximum of 100kb) and valid ID proof of present occupation in jpg/pdf format(maximum 200kb).
  •  A confirmation email will be sent to your provided email along with acknowledgement number which is important for further process.
  •  Visit library for payment of Rs. 1000.00 /-(new member) and Rs. 100.00 /-(Card renewal) along with working ATM card and acknowledgement number.
  • Documents :

  •  For new card an applicant must be provide a copy of any ID proof document, Address proof, Acknowledgement no. and Forwarding from Head of the Department. For card renewal only acknowledgement no. is necessary.

Basic Details


Your selected image must be either in .jpg or .jpeg format. The image resolution can be maximum of 80 × 100 pixels by width and height. Also the image must be of maximum 100 kb size.



Upload admission reciept for students, appontment letter for CIT staff/JRF and ID document of present organisation for Guest member. Document must be of maximum 200 kb size and .pdf/.jpg format.
